Print a daily or weekly prayer journal, a personal prayer list or a church prayer-request sheet, in your wording.

Printing sends only the sheet to the printer at 100% scale. The PDF is vector (sharp at any size); the PNG shows the first page. Everything is drawn in your browser — nothing is uploaded.

The layouts are this page’s own. The Christian verse is from the King James Version (public domain); the Muslim line is a plain rendering of the meaning of Qur’an 40:60. The PDF uses standard Latin fonts — for other scripts, download the PNG.

Runs entirely in your browser. Nothing you enter or upload is sent to a server.

Free prayer journal template and prayer list

This prayer journal template prints clean, practical pages for a personal prayer life or a church prayer team. Choose a daily prayer journal page with six ruled sections, a weekly page with a space for each day, a prayer list with a box to tick each day you pray and a column for the answer, or a prayer request sheet for a church, small group or women’s fellowship.

The wording can be neutral for any faith, Christian — with the ACTS prompts of Adoration, Confession, Thanksgiving and Supplication and a King James verse — or Muslim, for a du’a journal. Pick A4, US Letter, A5 or Legal, set the line spacing and colour, add a title and your group’s name, and print as many pages as you need or download a free PDF or PNG. Everything is drawn in your browser.

How to use a prayer journal

Keep it short and regular: a few lines each day are worth more than a page once a month. On the daily page, begin with what you are thankful for, write honestly in the reflection box, then list what you ask for yourself and for others. Note the verse or passage you read, and come back later to record answers — over weeks the journal becomes a record of how your prayers were met.

The ACTS pattern used in the Christian wording — Adoration, Confession, Thanksgiving, Supplication — is a common way to structure prayer; the Muslim wording follows gratitude, seeking forgiveness and du’a for yourself and others. The weekly page suits a prayer journal for women, men or teens who prefer one sheet a week.

Prayer list for church and small groups

Print the request sheet for a prayer meeting, a Sunday welcome table or a prayer chain. Each row has room for the date, the name, the request, how to follow up and when it was answered; the footer reminds everyone that requests are kept in confidence. Ask permission before sharing anyone’s request beyond the group.
