A baby milestones checklist you can tick, save and print
This baby milestones checklist brings together the CDC’s “Learn the Signs. Act Early.” checklists — the version revised with the American Academy of Pediatrics in 2022 — for every check-up age from 2 months to 5 years. Enter your child’s date of birth and the right checklist opens; mark each milestone Yes, Not sure or Not yet, and your answers are saved in this browser for next time.
You can print a tidy baby milestones chart for one age, with your answers ticked and space for questions, or all twelve checklists at once to keep in a folder or take to appointments.
Milestones describe what most children can do by an age. They are a conversation starter with your doctor, not a test your child passes or fails.
How to use the Baby Milestones Checklist
- Enter the date of birth, and if your baby was born early, how many weeks — the checklist for the corrected age opens.
- Or tap an age: 2, 4, 6, 9, 15, 18 or 30 months, or 1, 2, 3, 4 or 5 years.
- For each milestone, choose Yes, Not sure or Not yet.
- If anything is not yet or not sure, read the Act early note and raise it at your next visit.
- Print this checklist or all twelve, or download them as PDF or PNG.

Baby milestones by month: what changed in 2022
The CDC and AAP updated the checklists in 2022 (Zubler and colleagues, Pediatrics). Each milestone is now one that 75% or more of children reach by that age, instead of the average age, so a missing milestone is more meaningful. Checklists were added for 15 and 30 months, and some items, such as crawling, were removed because many children skip them.
| Age | Examples from the checklist |
|---|---|
| 2 months | Smiles when you talk to or smile at her; holds head up when on tummy |
| 4 months | Makes sounds like “oooo”, “aahh”; holds head steady without support |
| 6 months | Laughs; rolls from tummy to back |
| 9 months | Looks when you call her name; sits without support |
| 1 year | Waves “bye-bye”; pulls up to stand |
| 18 months | Tries to say three or more words besides “mama” or “dada”; walks without holding on |
| 2 years | Says at least two words together; kicks a ball |
| 3 years | Talks with you in conversation; uses a fork |
Baby milestones by week and for premature babies
Checklists are set at check-up ages, not weeks: 2 months is about 8–9 weeks, 4 months about 17 weeks and 6 months about 26 weeks. Between checklists, look at the previous one: skills keep developing week by week.
For a baby born before 37 weeks, count age from the due date until 2 years. The corrected age calculator shows it in detail, and the child growth percentile calculator tracks weight and length alongside.
When to act early
- Your child is not doing one or more milestones for their age.
- Your child has lost a skill they once had — words, babbling, eye contact, walking.
- You are worried about hearing, vision, how your child plays or how they relate to people.
- Ask the doctor about developmental screening; early support makes a real difference, and you do not need a diagnosis to ask.
Baby Milestones Checklist FAQ
What are the baby milestones by month?
The CDC checklists cover 2, 4, 6, 9, 12, 15, 18, 24 and 30 months and 3, 4 and 5 years. Choose an age above to see every milestone in social, language, thinking and movement skills.
My baby has not reached a milestone. Should I worry?
Each milestone is something at least 75% of children do by that age, so some children reach it a little later. Mark it Not yet and mention it to your doctor; they may suggest a developmental screening.
Is the baby milestones chart a screening test?
No. Screening uses validated questionnaires done with a professional. This checklist helps you notice and talk about development.
Should I use corrected age for milestones?
Yes, for babies born early, until about 2 years. Choose how many weeks early and the tool picks the checklist for the corrected age.
Where are my answers stored?
Only in your browser on this device. Nothing is uploaded; clearing your browser data removes them.
Where do the milestones come from?
They are the CDC “Learn the Signs. Act Early.” checklists (2022 revision with the AAP), reproduced word for word from cdc.gov as of October 2026. This page is not affiliated with or endorsed by the CDC.
